10 Barra $$ earns $10 Target Gift Card

GUIDELINES

• Barra $$ will not be given at emergency or repair visits
• The GOOD dental visit statement means no cavities or fillings needed
• Barra $ for excellent hygiene is based on each appointment and not any previous ones
• Barra $$  for a referral will be on a per family basis (ex. Only 5 extra $$ per family even if there are 2 or 3 children)
• Barra $$ is only for patients actively in treatment and not their family members
